ENTRANCE EXAMINATION FOR ADMISSION TO
POST GRADUATE MEDICAL COURSES (PGM), KERALA 2012

The Entrance Examination for admission to Post Graduate Medical
Courses (PGM), Kerala for the year  2012, will be conducted on 19.02.2012 
(Sunday) at Thiruvananthapuram, Ernakulam, Thrissur and Kozhikode.

Instructions for filling Online Application- JIPMER


Candidate should read the instructions carefully before making any entry or selecting options. 
Candidate should supply all the required details while filling up the online form. Mandatory fields 
are marked with * (asterisk) sign. The filling of online application contains two parts:
MD/MS Entrance Exam 2012:
Part I - New Application (Closes on 09-01-2012)
Part II - Complete Application (Closes on 09-01-2012 at 5:00 PM)
1. In Part I “New Application”, candidate will have to fill basic information. On submission of 
details, candidate will be prompted to check the details and make correction, if any, in the 
application. 
2. When the Next button is clicked, Applicants must have to upload a recent passport size 
photograph with file size not more than 100KB. (File size that exceeds the limit of 100KB 
will not be accepted).
3. When “Next” button is pressed, and after selecting the payment mode a page with 
Reference Number will be generated. Please note down reference number, user name 
and password or take a print out of the page. 
4. Reference number is must for future Correspondence.
5. Without the Reference number, No details of the Applicant can be retrieved.
6. Candidates who can pay application fee online though payment gateway can complete 
the Application Process in a step and get a copy of Application Printout immediately.
7. Candidates who selected the Demand Draft (DD) Payment mode must Login again to fill 
the Payment Details.
8. Part II “Complete Application” contains filing procedure of payment details –
9. Candidates who selected the Online Payment mode in Part I “New Application” and failed 
to pay online at the instance can continue online payment by Login again by  Part II 
“Complete Application”
10. Those who want to pay online, through net banking, credit card or debit card can use Part 
II “Complete Application” after completion of Part I “New Application”. 
11. Candidate will have to supply  reference number, user name, password and Name to 
continue to Part II “Complete Application”.
12. Candidates can take the Print out of the application 
13. Keep the Xerox copy of the application after filling all the details for any future reference.14. In case of DD Payment, DD  should be  drawn in favour of "Accounts Officer, JIPMER” 
payable at PONDICHERRY, Union Territory”
15. Failure to post this printout duly signed with the Bank DD, copies of 
OBC/SC/ST/OPH certificates (If Applicable) will lead to rejection of the application
16. Copy of the application along with the DD  should be sent to Professor (Academic), 
JIPMER, PUDUCHERRY - 605 006. 
17. Application should necessarily be accompanied with an attested copy of the OBC / SC / 
ST / OPH certificate (wherever applicable). The certificate should have been issued by 
the Competent Authority.
18. Application without the OBC / SC / ST / OPH certificate would be considered only under 
the General Category.
19. Change in category after submission of application would not be considered under any 
circumstances.
20. No communication would be entertained in this regard.

General Instructions to Candidates

Please ensure that you have the following, ready on hand before you start filling up the Online Application Form

Note: Send the Application and enclosures in A4 size envelope only. Do not fold the application form.

bDemand Draft drawn in favour of "COMEDK", payable at Bangalore for Rs.2000/- (Two Thousand Only).
bAll marks cards (First Year to Final Year university examination marks card/s)
bAttempt Certificate is a statement indicating the number of attempt one has taken to clear a particular subject/year.
bEnsure that the online application form is filled up properly and completely.
bAfter you have filled in your online application form, click on the Submit button only once, to send the details to the server.
bThe server will assign you an application number, and provide the facility to view your online application form with the details you have filled, along with the 6 digits application number.
bOnly after verifying the details take a print of the Application Form and the Date of Birth Certificate.
bThe completed application form must have the candidate’s signature, impressions of left thumb, left forefinger and left middle finger.
bKindly do not send any original documents along with the application form.
bKindly stick your recent (not older than six months) passport size colour photograph only in the space provided. Application Form with smudged / tampered or hazy photograph will be rejected.
bDo not staple or pin your photograph to the application form. Do not Sign. No attestation is required on the photograph.
bPlease adhere to the time schedule enlisted in the calendar of events.
bIncomplete / incorrect applications will be rejected without prior information to the candidate.
bMultiple application form received from the same candidate will result in confusion and may result in disqualification.
bDemand Drafts in excess of the prescribed fee (Rs.2000/-) if sent by the candidate will not be refunded.
bNon-refundable fee for taking COMEDK PGET-2012 and attending centralized counseling, if eligible, is Rs.2000/- (Rupees Two Thousand Only).
bCOMEDK is not responsible for any error the candidate makes while filling up the Application Form.
bCOMEDK will not entertain any changes in the application form and no refund will be made.
bCandidates who are listed in the Red List featured on our website need to contact COMEDK Contact No (080-41132810) for clarifications.
bCOMEDK is not responsible for any loss or damage which might occur during Postal or Courier delivery.

bDemand Draft drawn in favour of "COMEDK", payable at Bangalore for Rs.2000/- (Two Thousand Only) application number & applicant’s name to be written behind the DD. Check for the banker’s seal & signature on the DD.
bPrinted Online Application Form
The completed application form must have 
i. Candidate’s photo should be pasted (within the box provided).
ii. Candidate’s signature (within the box provided).
iii. Left thumb, forefinger and middle finger impression of the candidate (within the space provided).
bPrinted online Date of Birth Certificate / Identity Proof (COMEDK Copy)
The completed Date of Birth Certificate / Identity Proof must have
i. Attestation (within the space provided) by the Head of the Institution where he/she last studied or Gazetted officer not below the rank of Tahsildar of the area in which he/she is a resident.
ii. Candidate’s photo should be pasted (within the box provided).
iii.Candidate’s signature (within the box provided).
bPhotocopies of All marks cards including that of failed year if any (First Year to Final Year university examination marks card/s).
bPhotocopy of Attempt Certificate is a statement indicating the number of attempt one has taken to clear a particular subject/year.
bPhotocopy of Internship Completion Certificate / Internship Completed Certificate.
bPhotocopy of Degree Certificate/Provisional Degree Certificate.
bPhotocopy of Registration Certificate.
bCandidates belonging to SC / ST of All India and OBC of Karnataka would be considered, provided they furnish the duly attested Photocopy of the following Certificates:
a. SC / ST: Caste Certificate issued by competent authority (Authorised to issue such a certificate but not below the rank of a Tahsildar)
b. OBC: Category I, IIA, IIB, IIIA and IIIB: As notified by the Karnataka Govt. Caste and Income Certificate issued by Tahsildar or higher revenue jurisdictional authority authorised to issue such a certificate.
c. MINORITY: Religious (Christian) of All India / Linguistic (Tulu/Telugu) of Karnataka minority candidates to produce certificate as per Appendix –IV of Brochure (available on www.comedk.org)

Candidates who have not produced appropriate supporting documents for claiming the status of Religious / Linguistic Minority / SC / ST / OBC are liable to be considered as General Merit Candidates.

Note:
Avoid stapling of Demand Draft to the application form, instead use clips.

Note:
The recent passport size photographs have to be identical. It should not be more than 6 months old. Application with smudged/tampered or hazy photograph will not be accepted. Any application received not adhering to these specifications will be rejected without any clarifications and no refund will be made. The decision of the COMEDK Committee on this would be final and binding.


The completed Application Form has to reach COMEDK office by speed post or courier on or before Friday the 13th January 2012 (before 5:00 PM) to the following address.
